Release of Liability clause for The Transformation Experience:
SECTION ONE: THIS IS A RELEASE OF LIABILITY.
In consideration of being an attendee in a Transformation Experience Event, the undersigned acknowledges, and agrees that:
I understand that The Transformation Experience is a nonprofit North Carolina non-profit organization, hereinafter referred to as The Transformation Experience. The Transformation Experience does not charge for prayer of deliverance ministry that I may receive. Any fees paid by me cover the cost for the materials, preparation and teachings that I receive at this event and is not tax deductible; and, I recognize that The Transformation Experience is an equipping and deliverance ministry and that their events are informative and are not designed as, or a substitute for, medical treatment or psychological therapy. I also acknowledge that these events address spiritual and life issues in an honest and frank way. Some may find this difficult, unsettling, mentally, and emotionally taxing and even physically stressful. I acknowledge that I am in good physical and emotional health and am unaware of any deficiency which would prohibit my participation in these events and, I acknowledge that The Transformation Experience staff and team members have voluntarily agreed to pray for me. I understand that any deliverance session is a prayer ministry. I understand that while some of The Transformation Experience staff are ordained ministers, none of the them are licensed counselors and that the retreat is not a professional counseling session; and, I further state that I have voluntarily sought assistance of my own initiative and I am under no obligation to accept any of the advice or help that I might receive from any staff or team member of The Transformation Experience. Therefore, I take responsibility for my own life and any application of the prayer ministry or information I receive at this retreat and can make the choice to opt out of any activity at any time for any reason; and, I understand by participating in The Transformation Experience retreat I may be photographed for print, video or other electronic imaging for use in promotional materials and other formats both electronic and published. I understand that the staff at The Transformation Experience is committed to my right to privacy and will not use any images with a resemblance to me without my written consent (see below). I also understand that in respect to privacy, that I will not share others’ information outside the event weekend. I understand that recording audio or video is not allowed on premises to protect the privacy of all; and, If any information that I share at this retreat poses a danger to myself or others, the staff at The Transformation Experience is not bound by confidentiality and will notify the proper authorities; and, I knowingly and freely assume all risks, both known and unknown, and assume full responsibility for my participation; and, I understand that The Transformation Experience staff and team members assumes no liability whatsoever for any damage, loss, or theft by third parties to the personal property of any participant at any The Transformation Experience event; and, I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
SECTION TWO: COVID -19 WAIVER AGREEMENT
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing and wearing masks.
I further acknowledge that The Transformation Experience has put in place preventative measures to reduce the possibility of a spread of the Coronavirus/COVID-19. I hereby has been made aware that:
Every ministering Transformation Vessel will participate in Temperature checks twice daily.
Every ministering Transformation Vessel will practice period handwashing and hand sanitizing.
The Transformation Experience Team will do their part to insure your safety by sanitizing the main meeting and lounging areas prior to each session and as well as sanitize periodically between each session.
I further acknowledge that The Transformation Experience cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, those ministering to me this weekend.
I voluntarily participate in The Transformation Experience and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending the Transformation Experience week-end event.
* I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold the Transformation Experience from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of The Transformation Experience or that may otherwise arise in any way in connection with any ministry received from The Transformation Experience. I understand that this release discharges The Transformation Experience from any liability or claim that I, my heirs, or any personal representatives may have against the Transformation Experience with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any ministry received from The Transformation Experience. This liability waiver and release extends to the The Transformation Experience together with all ministering vessels.
Electronic Signature Authorization
By applying my electronic signature to this agreement, I agree that my electronic signature is the legally binding equivalent of my handwritten signature on paper. I will not, at any future time, claim that my electronic signature is not legally binding or enforceable. By electronically signing and submitting this agreement, I 1) acknowledge that I have read and fully understand the terms of the agreement; 2) voluntarily agree to be bound by this agreement; and 3) certify that I am 18 years of age or older. My signature applies to all pages of this contract. I understand that if I wish to sign a hard copy of this agreement instead of an electronic version, I must contact the party that requires my signature on this agreement directly at :
